The 'Equine Wellness Program' is designed to provide you with the peace of mind that comes with knowing that your horse(s) are receiving the best preventative health care on a regular and timely basis.  Twice yearly, medical evaluations and preventative care services are provided for your horse(s) by Dr. Bennett.

Equine Wellness Program

Polk Equine, P.L would like to offer you and your horse the highest quality preventative health program.  We offer two levels of protection for your horse including the following services:

Program 1-

• Semi-annual or as labeled immunizations (EWT/Rhino, West Nile, Rabies).
• Coggins Test (EIA)
• Minor Exam (twice per year)                                        
• Dental Exam and Routine Float (if necessary)
• Fecal Test/ Sheath Cleaning
• Four (4) paste dewormers

Program 2 –

• Semi-annual or as label immunizations (EWT/Flu-Rhino, West Nile, Rabies, Strangles)
• Coggins Test (EIA)  
• Routine exam (twice per year)
• Routine Blood Count (CBC/Chemistry/Fib)
• Dental Exam and Routine Float (if necessary)
• Fecal Test/ Sheath Cleaning
• Strongid C Daily Wormer and two (2) paste wormers
• Contact office for rates

All appointments will be grouped by geographic area.  A representative of the owner is requested to be present at all visits.

Dewormers will be mailed directly to you via UPS/FedEx.

Additional preventative services will be discounted for enrolled patients (not including emergency visits).  However, arrangements must be made prior to scheduled visit due to time constraints.

Pro-rated refunds may be made in the event of sale or death of horse.  All program fees will be paid in advance.  Fees due February 16^th.  Monthly recurring credit card program available.

Three or more horses per owner will be discounted additional 5% per horse.
